Passports for Children Under 16
 
  1. Complete a passport application formComplete and print your online application.  
  2. Make an appointment.
  3. Submit the application and required supporting documents (see below for details) to the American Citizen Services section between 7:30 a.m. to 11 a.m. or 1 p.m. to 2 p.m., Monday through Friday.  You must sign your application before a Consular Officer.

Supporting Documents

  • Proof of Citizenship (Please bring one of the following original documents). You can submit your current passport as proof of both citizenship and identification.:
  • Previously issued U.S. passport;
  • U.S. birth certificate (original or copy certified by an official government custodian of birth records);
  • Naturalization Certificate
  • Certificate of Citizenship; or
  • Consular Report of Birth Abroad

Proof of Identity (Please bring one or more of the following original documents):

  • Previously issued U.S. passport;
  • Naturalization Certificate; or
  • A government-issued photo identity document issued by a competent authority of the municipality, state, or country in which the applicant resides (e.g. driver's license, or military I.D. card)

Evidence of relationship: The parent(s)/guardian(s) must submit evidence of their relationship to the minor applicant (one of the following):

  • Minor’s certified U.S. birth certificate with both parents’ names
  • Minor’s certified Foreign Birth Certificate with both parents’ names (this should be accompanied by an official English translation)
  • Minor’s Report of Birth Abroad with both parents’ names
  • Adoption Decree with adopting parents’ names (if not in English, this should be accompanied by an official English translation)
  • Court Order establishing custody
  • Court Order establishing guardianship

Notes:

  • Previous U.S. passports are not acceptable as evidence of relationship
  • Evidence of a legal name change must be submitted, if the name of a parent/guardian has changed since the original documents were issued (e.g. marriage certificate, etc.)

Present Identification of Parent(s)/Guardian(s) at the time of application (one of the following):

  • Previously issued, undamaged U.S. passport
  • Naturalization Certificate
  • Valid Driver’s License
  • Current Government Employee ID (city, state or federal)
  • Current Military ID (military and dependents)

Provide Parental Consent: Both parents must provide consent authorizing passport issuance for a minor under age 16.  See the scenarios below, and follow the instruction that best applies to your circumstance:

Both parents must:

  • Appear in person with the minor and
  • Sign Form DS-11 in front of a Consular Officer/Associate

One parent must:

  • Appear in person with the minor and
  • Sign Form DS-11 in front of a Consular Officer/Associate and
  • Submit the second parents’ notarized Statement of Consent (Form DS-3053)

One parent (with sole legal custody) must:

  • Appear in person with the minor and
  • Sign Form DS-11 in front of a Consular Officer/Associate and
  • Submit primary evidence of sole authority to apply for the child with one of the following:
    • Minor’s certified U.S. or foreign birth certificate listing only the applying parent
    • Consular Report of Birth Abroad (Form FS-240) or Certification of Birth Abroad (Form DS-1350) listing only the applying parent
    • Court order granting sole custody to the applying parent (unless child’s travel is restricted by that order)
    • Adoption decree (if applying parent is sole adopting parent)
    • Court order specifically permitting applying parent’s or guardian’s travel with the child
    • Judicial declaration of incompetence of non-applying parent
    • Death certificate of non-applying parent

A Third Party (in Loco Parentis applying on behalf of a minor under the age of 16) must:

  • Submit a notarized written statement or affidavit from both parents or guardians authorizing a third-party to apply for a passport
  • When the statement of affidavit is from only one parent/guardian, the third-party must present evidence of sole custody of the authorizing parent/guardian.

The child must prove his/her identity.  If the child is easily identifiable by the consular officer by looking at the previous passport, no additional proof may be necessary.  However, a child’s appearance may change significantly over time, so the consular officer may ask for additional proof of identity.  Parents should be prepared to provide photos of the child at each year of age from the time the last passport was issued until the present to facilitate the consular officer's identification of the child. 

Provide Social Security number on application

Photographs

Two identical photographs are required. They must be recent, 2 x 2 inches (with the size of the head between 1 and 1 3/8 inches), in color. The photos must be clear, front view, full face, and in focus. The background must be plain white or off-white. Please print the applicant's first and last names on the backs of both photos.

Pay the required fees:

Under Age 16: The passport application fee is $80.  The execution fee is $25.  The total is $105.

First adult passport (age 16 or over), replacement for a lost, stolen or mutilated passport of an adult: the passport application fee is $110.  The execution fee is $25.  The total is $135.

Fees may be paid in U.S. Dollars, Thai Baht, or by credit card.

Allow two weeks for processing. Passports must be picked up at the ACS section by the applicant or authorized representative.
