Passports
Full and limited validity passports are issued by the American Citizen Services section. In Thailand, all persons seeking a U.S. passport must appear in person at the Embassy to apply. To make an appointment for a passport service, please click here.
When traveling, please remember to carry a photocopy of your passport separately from your passport. In the event of loss or theft, the copy will help facilitate issuance of an emergency passport.
Procedure
1. Complete a passport application form. There are two possible forms (DS-11 and DS-82), depending on your individual circumstance. Use form DS-82 only if your previous U.S. passport:

Was issued when you were age 16 or older and
-
Was issued within the last 15 years and
-
Is undamaged and can be submitted at the time of application and
-
Was issued in your current name or you can legally document your name change.
If you answered no to any of the above statements you are not eligible to use form DS-82! You must submit form DS-11. Also, if the passport you are applying for is a replacement for a lost, stolen or mutilated passport you must use form DS-11.

2. Submit the application and required supporting documents (see below for details) to the American Citizen Services sefction between 7:30 a.m. to 11 a.m. or 1 p.m. to 2 p.m., Monday through Friday. You must sign your application before a Consular Officer.
Supporting Documents
A. Proof of Citizenship (Please bring one of the following original documents). You can submit your current passport as proof of both citizenship and identification.:

Previously issued U.S. passport;
-
U.S. birth certificate (original or copy certified by an official government custodian of birth records);
-
Naturalization Certificate
-
Certificate of Citizenship; or
-
Consular Report of Birth Abroad
B. Proof of Identity (Please bring one or more of the following original documents):
-
Previously issued U.S. passport;
-
Naturalization Certificate; or
-
A government-issued photo identity document issued by a competent authority of the municipality, state, or country in which the applicant resides (e.g. driver's license, or military I.D. card)
C. Provide Social Security number on application
D. Photographs
Two identical photographs are required. They must be recent, 2 x 2 inches (with the size of the head between 1 and 1 3/8 inches), color or black and white. The photos must be clear, front view, full face, and in focus. The background must be plain white or off-white. Please print the applicant's first and last names on the backs of both photos.
E. For minors (children under 16 years of age), please see below for additional requirements. The below requirements must be met.
3. Pay the required fees: $85 for a minor under the age of 16, $75 for an adult renewal, or $100 for an adult first-time issuance or replacement for a lost, stolen, or mutilated passport. Fees may be paid in U.S. Dollars, Thai Baht, or by credit card.
4. Allow two weeks for processing. Passports must be picked up at the ACS section by the applicant or authorized representative.
Lost or Stolen Passports
The loss of a valid U.S. passport is a serious matter. After searching carefully, back-tracking the places where it may have been lost, such as stores and banks, and checking with the appropriate Lost and Found departments and transportation companies, report the loss to a police station close to where you believe the loss took place; or to any Tourist Police office. The Tourist Police telephone hotline number is 1155. To replace the passport, you must apply in person at the U.S. Embassy during regular office hours, with the police report. A photocopy of the face page of the lost passport is helpful, as is photographic identification and other proof of U.S. citizenship. Acceptable evidence of citizenship can be an original or certified copy of your U.S. birth certificate, an original or certified copy of a Consular Report of Birth, a Naturalization Certificate, or a previous expired passport.
It normally takes 24-48 hours to replace a lost or stolen passport, but can take longer if you lack these documents. The fee for replacement of a lost or stolen passport if $85.00 for minors under age 16 and $100 for adults age 16 and older. Fees may be paid in U.S. Dollars, Thai Baht, or by credit card.
Please note:
1. Applicants must present the original Thai police report concerning the circumstances of the loss or theft.
2. Applicants must complete form DS-64 Statement Regarding a Lost or Stolen Passport. This form is available here or at the ACS Office.
3. Applicants are normally required by the Royal Thai Government to obtain a replacement entry stamp in their passport. The ACS section will provide information on how to accomplish this when you come to obtain a replacement passport.
Emergency Passports
Emergency passports are issued on a case by case basis, and are valid for one year only. Emergency passports require a minimum of 24 hours to process. We are prohibited from adding additional visa pages to an emergency passport.
Extra Visa Pages
To obtain additional visa pages, you must complete an Application for Additional Visa Pages (DS-4085: Amendment/Validation Application). Please complete the form using black ink and submit it with your passport to the American Citizen Services section at the U.S. Embassy. U.S. passport holders 16 and older must personally sign the DS-4085 but do not need to come to the Embassy personally to request new pages. Rather they can send a person to do this for them, with a signed statement specifically allowing the designated person to do this on their behalf. The designated person will need to bring valid photo identification.
For children under 16, one parent needs to come and sign the DS-4085 at the Consular Section of the Embassy with his/her passport. On that form, the parent should indicate his/her relationship next to the signature on the form. Though not required, the consul may ask to see an original or certified copy of the child’s birth certificate showing both parents names. The child does not have to appear at the Embassy.
In cases where all visa pages of a valid passport have been used, extra pages can be added to your passport at no cost, while you wait. Please allow one hour for this process.
Passport Expiration Dates
Several American Citizens have reported to our office that they have been denied boarding at the airport for travel to third countries from Thailand due to the expiration date in their passport being under six months from the date of travel. This has also happened to travelers coming to Thailand. Please be aware that many countries require six full months of validity remaining on a passport in order to be granted a visa on arrival.
Applications for Minor Children
In addition to proof of citizenship and proof of identity, the following must also be presented at the time of application.
--Evidence of relationship: The parent(s)/guardian(s) must submit evidence of their relationship to the minor applicant (one of the following):

Minor’s certified U.S. birth certificate with both parents’ names
-
Minor’s certified Foreign Birth Certificate with both parents’ names (this should be accompanied by an official English translation)
-
Minor’s Report of Birth Abroad with both parents’ names
-
Adoption Decree with adopting parents’ names (if not in English, this should be accompanied by an official English translation)
-
Court Order establishing custody
-
Court Order establishing guardianship
Notes:
-
Previous U.S. passports are not acceptable as evidence of relationship
-
Evidence of a legal name change must be submitted, if the name of a parent/guardian has changed since the original documents were issued (e.g. marriage certificate, etc.)
--Present Identification of Parent(s)/Guardian(s) at the time of application (one of the following):

Previously issued, undamaged U.S. passport

Naturalization Certificate

Valid Driver’s License
-
Current Government Employee ID (city, state or federal)

Current Military ID (military and dependents)
--Provide Parental Consent: Both parents must provide consent authorizing passport issuance for a minor under age 16. See the scenarios below, and follow the instruction that best applies to your circumstance:
Both parents must:
• Appear in person with the minor and
• Sign Form DS-11 in front of a Consular Officer/Associate
One parent must:
• Appear in person with the minor and
• Sign Form DS-11 in front of a Consular Officer/Associate and
• Submit the second parents’ notarized Statement of Consent (Form DS-3053)
One parent (with sole legal custody) must:
• Appear in person with the minor and
• Sign Form DS-11 in front of a Consular Officer/Associate and
• Submit primary evidence of sole authority to apply for the child with one of the following:
-Minor’s certified U.S. or foreign birth certificate listing only the applying parent
-Consular Report of Birth Abroad (Form FS-240) or Certification of Birth Abroad (Form DS-1350) listing only the applying parent
-Court order granting sole custody to the applying parent (unless child’s travel is restricted by that order)
-Adoption decree (if applying parent is sole adopting parent)
-Court order specifically permitting applying parent’s or guardian’s travel with the child
-Judicial declaration of incompetence of non-applying parent
-Death certificate of non-applying parent
A Third Party (in Loco Parentis applying on behalf of a minor under the age of 16) must:
• Submit a notarized written statement or affidavit from both parents or guardians authorizing a third-party to apply for a passport
• When the statement of affidavit is from only one parent/guardian, the third-party must present evidence of sole custody of the authorizing parent/guardian.
For children under the age of 6, parents should be prepared to provide photos of the child at each year of age to facilitate the consular officer's identification of the child from infancy. The Consular Officer may request this information of older applicants as well.
