Passports

Full and limited validity passports are issued by the American Citizens Services Unit.   In Thailand, persons seeking a U.S. passport must appear in person at the Embassy to apply. To make an appointment for a passport service, please click here.

For further information you may refer to the U.S. Department of State website on passport applications. Please remember that some procedures for passport applications abroad may vary according to the country in which the passport application is made.

When traveling, please remember to carry a photocopy of your passport separately from your passport. In the event of loss or theft, the copy will facilitate issuance of an emergency passport.

Documentary Requirements

1. Proof of Citizenship (Please bring one of the following original documents):

  • Previously issued U.S. passport;
  • U.S. birth certificate (original or copy certified by an official government custodian of birth records);
  • Certificate of Naturalization;
  • Consular Report of Birth Abroad

2. Proof of Identity (Please bring one or more of the following original documents):

  • Previously issued U.S. passport;
  • Certificate of Naturalization;
  • A formal identity document issued by a competent authority of the municipality, state, or country in which the applicant resides (e.g. driver's license)

3. Photographs
           
            Two identical photographs are required. They must be recent, 2 x 2 inches (with the size of the head between 1 and 1 3/8 inches), color or black and white. The photos must be clear, front view, full face, and in focus. The background must be plain white or off-white. Please print the applicant's first and last names on the backs of both photos.

Procedure

On February 1, 2008, the U.S. Government will increase its fees for certain U.S. citizen passport services.  For adult applicants renewing a passport, the total fee will increase to $ 75, and replacement for a lost, stolen, or mutilated passport will also increase to $100.  For first-time applicants age 16 and over, the total fee will increase to $ 100.  The fee for minors under 16 years of age will be $ 85.  Fees may be paid by in U.S. Dollars, Thai Baht, or by debit or credit card.

Please refer to the Department of State’s website at www.travel.state.gov for complete information on all new requirements, including downloadable revised forms that must be used as of February 1, and the U.S. Embassy website at http://bangkok.usembassy.gov, for complete U.S. passport application instructions.

1. Complete a passport application form (DS-11 or DS-82 depending on your situation).  Forms are available at the ACS office. Use passport application form DS-82 only if:

  • you have been issued a U.S. passport in your name within the past 15  years;
  • are able to submit your most recent U.S. passport at the time of application;
  • you were at least 16 years of age when your most recent passport was issued.
  • In cases of name change, you can submit proper documentation.

If the passport you are applying for is a replacement for a lost, stolen or mutilated passport you must use form DS-11.

2. Submit the application and required supporting documents to the American Citizen Services Office between 7:30 a.m. to 11 a.m. or 1 p.m. to 2 p.m., Monday through Friday. You must sign your application before a Consular Officer.

3. Pay the required fees: $85 for a minor application, $75 for an adult renewal, or $100 for an adult first-time issuance or replacement for a lost or stolen passport. Fees may be paid by in U.S. Dollars, Thai Baht, or by debit or credit card.

4. Allow two weeks for processing. Passports must be picked up at the ACS section by the applicant or authorized representative. 

Applications for Minor Children

For children under 16 years of age, the consent of both parents is mandatory. If one parent is not present in the country of application, he or she may complete and have notarized Form DS 3053 and send this form to the other parent granting permission for the application and issuance of a passport to the child. In addition, a certified birth certificate and photocopy are required as evidence of the child's relationship to the parents/guardians.

Effective February 1, 2008, passport applicants who are U.S. citizen minors under the age of 16 must appear personally with their parents to establish identity, proof of citizenship and proof of relationship.  For parental application permission, both parents must appear together and sign or one parent may appear to sign and submit the other parent’s notarized statement, or one parent may appear, sign, and submit primary evidence of sole authority.  Minors age 16 and 17 may apply in person with their own identification, but for security reasons, parental consent may be required to accompany the child and parent identification.
 
For children under the age of 5, parents should be prepared to provide photos of the child at each year of age to facilitate the consular officer's identification of the child from infancy.  The Consular Officer may request this information of older applicants as well.

Lost or Stolen Passports

1. Applicants must present the original Thai police report concerning the circumstances of the loss or theft.
 
2. Applicants must complete an Affidavit of Loss or Theft (available at the ACS Office).

3. Applicants are normally required by the Royal Thai Government to obtain a replacement entry stamp in their passport.  The ACS section will provide information on how to accomplish this.

Emergency Passports

Emergency passports are issued on a case by case basis, and are valid for one year only.  Emergency passports require a minimum of 24 hours to process. 

Extra Visa Pages

To obtain additional visa pages, you must complete an Application for Additional Visa Pages (DS-4085: Amendment/Validation Application).  Please complete the form using black ink and submit it with your passport to the American Citizen Services section at the U.S. Embassy.  U.S. passport holders 16 and older must personally sign the DS 4085 but need not come to the Embassy personally to request new pages. 

For children under 16, one parent needs to come and sign the DS-4085 at the Consular Section of the Embassy with his/her passport. On that form, the parent should indicate his/her relationship next to the signature on the form. Though not required, the consul may ask to see an original or certified copy of the child’s birth certificate showing both parents names. The child does not have to appear at the Embassy.

In cases where all visa pages of a valid passport have been used, extra pages can be added to your passport at no cost, while you wait.  Please allow one hour for this process.

Passport Expiration Dates

Several American Citizens have reported to our office that they have been denied boarding at the airport for travel to third countries from Thailand due to the expiration date in their passport being under six months from the date of travel.  Please be aware that many countries require six full months of validity remaining on a passport in order to be granted a visa on arrival.
