Lost or Stolen Passports
 

The loss of a valid U.S. passport is a serious matter.  After searching carefully, back-tracking the places where it may have been lost, such as stores and banks, and checking with the appropriate Lost and Found departments and transportation companies, report the loss to a police station close to where the loss took place; or to any Tourist Police office.  The Tourist Police telephone hotline number is 1155.

To replace the passport, you must apply in person at the U.S. Embassy during regular office hours, with the police report. A photocopy of the face page of the lost passport is helpful, as is photographic identification and other proof of U.S. citizenship.  Acceptable evidence of citizenship are: 

  • an original or certified copy of a U.S. birth certificate,
  • an original or certified copy of a Consular Report of Birth,
  • a Naturalization Certificate,
  • a previous or expired passport. 

PROCEDURE TO APPLY FOR A PASSPORT

  1. Report the loss to the police.  Report the loss or theft of your passport to a police station close to where the loss took place, or to any Tourist Police office.  The Tourist Police telephone hotline number is 1155.  You must bring this police report with you to the U.S. Embassy when you apply for your replacement passport.
  2. Complete a passport application form and DS-64 Statement Regarding a Lost or Stolen Passport.  Complete and print those forms here
  3. Make an appointment (click here), suggested but not required.
  4. Submit the application and required supporting documents (click here for details) to the American Citizen Services section between 7:30 a.m. to 11 a.m., and 1 p.m. to 2 p.m., Monday through Friday, except US and Thai holidays.  You must sign your application before a Consular Officer.
  5. Pick up your new Passport:  No appointment required. • Emergency passports are issued on a case by case basis, with validity period of three months up to one year, depending on circumstances. You will need to present evidence, such as a flight itinerary, to prove you have a genuine need for an emergency passport.  Emergency passports require a minimum of 24 hours to process.  We are prohibited from adding additional visa pages to an emergency passport.
     
    • Allow two weeks for processing, if there is no urgent need for your passport. Passports must be collected at the ACS section by the applicant or authorized representative, (who will need to present a signed letter of authorization and photo identification)
  6. Obtain a replacement entry stamp.  The Royal Thai Government requires a replacement entry stamp  be obtained for the new passport.  After  collection of the replacement passport, you must present it with the police report to the Thai Immigration Bureau, where a replacement entry stamp will be issued.  Departure from Thailand will most likely be denied without the replacement stamp.  The Thai Immigration Bureau is located at Government Center Chaengwattana Building B, No. 120, Moo 3, Chaengwattana Road, Tungsonghong Sub-District, Laksi District, Bangkok 10210. PASSPORT EXPIRATION DATES

Please be aware that many countries require six full months of validity remaining on a passport in order to issue a visa upon arrival. Boarding may be denied at the airport for travel to third countries from Thailand, and travel back into Thailand if -there is less than six months validity remaining.
